New Client Forms
Welcome to the Team! I am so excited to begin working with you as your Personal Fitness and Nutrition Coach.
For your convenience, links to all of our initial paperwork are included below.
Some are requirements before we get started (The Official Stuff), and others are more fun to help me get to know you. They will also help us with setting goals together. Altogether they should take you less than 30 minutes to complete.
Once I receive the completed forms, we can get started!
If you have any questions, just ask: katie@katiestauffertraining.com

Getting to Know You:
About your Coach:
I’m asking you to share a lot of information about yourself, so I thought it might be nice to know a little more about me!
By way of background, I hold a BA degree in Spanish, with a minor in Zoology, and have a Masters in Healthcare Administration. In the fitness related category, I am a NASM Certified Personal Trainer as well as a Certified Nutrition Coach. I am also a Certified Women’s Coaching Specialist through Girls Gone Strong and hold a certification in Online Personal Training from Fitness Mentors. I have a life-long passion for learning and growing - and I hope to share that passion and knowledge with everyone I coach.
I know from personal experience what it is like to try to balance work, family, life and exercise. Before leaping into becoming a fitness professional, I worked as a product manager in the healthcare industry. Building my passion for fitness while managing work and family gave me many opportunities to learn through trial and error the value of balancing hard work with care for myself, and just making time to enjoy the ride. These experiences have made me ever more passionate to help other women (and men!) find the time to prioritize their own strength, health and wellness in the midst of every day responsibilities.
I have been sort of a jack of all trades when it comes to athletics and fitness. I have been everything from a competitive rower and then Olympic weightlifter in my younger days, to a long distance runner and triathlete. Once I had kids I had to put so many things into perspective and find better balance. I am focused these days on building sustainable healthy habits, functional strength and all around energy for family and life adventures.
I also confess to being a lover of good beer, wine, and delicious food - many of my adventures include a good brewery or winery outing. I’m a tiny bit of a sci-fi nerd (Star Trek anyone?) and also love movies, especially 80s comedies – until recently we even had a working VCR connected to our smart TV, the way all essential 80s movies were meant to be viewed!
Feel free to follow me on Facebook or Instagram (@katiestauffertraining - also linked at the bottom of the page) and check out the blog here on the site. I will admit to being less consistent in my posts than I would like, but I am committed to always posting helpful information when I do. And if you have a topic you would like to hear about, please let me know!
I hope you are ready for a challenging yet fun journey as we work together to help you achieve your health and fitness goals. Again, welcome to the team!
